metadata.dc.description.resumo: A Fibrilação Atrial (FA) é arritmia cardíaca sustentada mais comum, está associada com elevadas taxas de morte, AVE, insuficiência cardíaca, disfunção ventricular e diminuição da qualidade de vida. Hipertensão arterial pode estar presente em até 70% dos pacientes com FA. Pacientes com FA geralmente recebem terapia anticoagulante para prevenir o risco de AVE, mas que por outro lado pode aumentar o risco de hemorragia intracraniana. Tanto isquemia, quanto hemorragia cerebrais, podem levar ao AVE e aumentar a pressão intracraniana (PIC). A hipertensão intracraniana tem efeito na perfusão cerebral, o que pode resultar também em isquemia ou lesão cerebral secundária. Esse estudo teve objetivo avaliar a PIC em dois grupos pacientes: com fibrilação atrial (FA) e grupo controle sem fibrilação atrial (CON) por meio de uma metodologia não-invasiva. A metodologia para a avaliação da PIC consistiu em um sistema externo de chips que transmite, em tempo real, sinais elétricos de deformação craniana a um monitor. Nesse trabalho observamos que o grupo FA apresentou idade (: 0,004), glicemia de jejum, hemoglobina glicosilada (HbA1c), ureia e creatinina maiores do que o grupo controle. Não houve diferença quanto ao gênero e nem em relação à PA. O número de pessoas com PIC alterada foi de 58,8% no grupo FA e 46,7% no grupo CON (:0,7393); O grupo FA apresentou alterações mais intensas que o grupo controle (:0,013). A PIC correlacionou-se diretamente com PAS grupo CON, mas não com a PA do grupo FA. A avaliação não invasiva da PIC apresenta-se como uma excelente ferramenta, à medida que foi capaz de discriminar a intensidade da relação P2/P1 entre os dois grupos. No entanto, mais estudos são necessários para entender o significado da alteração da PIC em pacientes com FA.
Abstract: Atrial fibrillation (AF) is the most common sustained cardiac arrhythmia associated with high death rates, stroke, heart failure, ventricular dysfunction, and decreased quality of life. Arterial hypertension may be present in up to 70% of patients with AF. Patients with AF generally receive anticoagulant therapy to prevent the risk of stroke, but on the other hand may increase the risk of intracranial hemorrhage. Both ischemia and cerebral hemorrhage can lead to stroke and increase intracranial pressure (ICP). Intracranial hypertension has an effect on cerebral perfusion, which may also result in ischemia or secondary brain injury. The aim of this study was to evaluate ICP in two patient groups: atrial fibrillation (AF) and control group without atrial fibrillation (CON) using a noninvasive methodology. The methodology for the evaluation of PIC consisted of an external chip system that transmits, in real time, electrical signals of cranial deformation to a monitor. In this study, we observed that the FA group presented age (: 0.004), fasting glycemia, glycosylated hemoglobin (HbA1c), urea and creatinine higher than the control group. There was no difference regarding gender or BP. The number of people with altered ICP was 58.8% in the AF group and 46.7% in the CON group (: 0.7393); The AF group presented more intense alterations than the control group (: 0.013). The ICP was directly correlated with the SBP group, but not with the AP of the FA group. The noninvasive evaluation of ICP is an excellent tool, as it was able to discriminate the intensity of the P2 / P1 relationship between the two groups. However, further studies are needed to understand the significance of PIC change in patients with AF.
