metadata.dc.description.resumo: | Pacientes hospitalizados com COVID-19 são susceptíveis a diversas complicações associadas a gravidade da doença e dentre elas estão as alterações bucais. As hipóteses sugerem que as manifestações bucais nestes pacientes podem estar associadas a complicações sistêmicasda doença ou fatores locais de replicação viral. A partir desta perspectiva, considerando a hipótese de que pacientes com COVID-19 manifestam lesões bucais associadas a condição sistêmica, o objetivo deste estudo é investigar a associação entre lesões bucais de pacientes hospitalizados e a COVID-19; bem com analisar se estas lesões bucais podem ser consideradas preditoras de evolução para óbito em pacientes hospitalizados com COVID-19. Trata-se de um estudo caso- controle, em que os pacientes foram avaliados no Hospital Universitário Regionaldos Campos Gerais (HURCG), em Ponta Grossa, PR, Brasil, sendo incluídos pacientes com teste RT-PCR positivo para COVID-19 ou pacientes com sintomas gripais, porém com teste RT-PCR negativo (grupo controle). Os indivíduos receberam exame físico bucal voltado para identificação de lesões bucais com registro fotográfico, coleta de conteúdo salivar para investigar Cálcio e Fosfatase salivar. Através de prontuário eletrônico foram avaliados dados referentes aos dados sociodemográficos e exames hematológicos. A amostra final incluiu 112 pacientes sendo 69 pacientes com teste positivo e 43 pacientes com teste negativo para COVID-19. Os resultados evidenciaram associações entre a presença de lesões bucais e a hospitalizaçãopor COVID-19, sendo que as lesões associadas a doença foram “úlceras hemorrágicas”,“úlceras traumáticas” e “queilite angular”. Concluímos que as lesões bucais em pacientes hospitalizados foram associadas à COVID-19, e estas indicam um risco 13 vezes maior do indivíduo evoluir ao óbito. |
Abstract: | Patients hospitalized with COVID-19 are susceptible to several complications associatedwith disease severity, including oral changes. The hypotheses suggest that oral manifestations in these patients may be associated with systemic complications of the disease or local viral replication factors. We consider the hypothesis that patients with COVID-19 manifest oral lesions related to the systemic condition. This study aims to investigate the association between oral lesions of hospitalized patients and COVID-19 and analyze whether these may be considered predictors of evolution to death in patients with COVID-19. We carried out a case- control study in which patients were evaluated at Hospital Universitário Regional dos Campos Gerais (HURCG) in Ponta Grossa, PR, Brazil. We included patients with positive RT-PCR test for COVID-19 or patients with influenza symptoms but with negative RT-PCR test (control group). The subjects received an oral physical examination to identify oral lesions with a photographic record collection of salivary content to investigate calcium and salivary phosphatase. Data regarding sociodemographic data and hematological exams were evaluated through electronic medical records. The final sample included 112 patients, with 69 patients testing positive and 43 testing negative for COVID-19. The results showed an association between the presence of oral lesions and hospitalization for COVID-19, and the lesions associated with the disease were "hemorrhagic ulcers", "traumatic ulcers", and "angular cheilitis". We conclude that oral lesions in hospitalized patients are associated with COVID- 19, and these indicate a 13 times greater risk of the individual evolving to death. |
